1. Must be a U.S. resident and 18 years of age to purchase and/or sell tickets. Only one (1) individual may be identified as the purchaser (or buyer) of the raffle ticket.

2. Tickets may be purchased via the Internet, at Developmental Services Center (1304 W. Bradley Ave., Champaign, IL) by calling Developmental Services Center’s Development Office (217-356-9176), by mail (1304 W. Bradley Ave., Champaign, IL 61821) at designated ticket outlets or from raffle salespersons. The Developmental Services Center’s Development Office will e-mail a verification letter to the primary registered ticket buyer once your check or credit card has been processed. Please allow one week to receive your confirmation. If said notice is rejected and returned to Developmental Services Center (DSC) no further attempt will be made to deliver the notice. DSC and its volunteers assume no liability for lost, late, misdirected, illegible or undelivered entries.

3. If there is more than one name on an entry, the prize will be awarded to the first name listed on the entry as the winner, as long as that person is 18 years old or older and a U.S. resident. It is the sole responsibility of the winner in a multiple party entry to allocate any prize among the participants in that entry.

4. In the event that a minimum of 1,500 tickets are not sold by September 17, 2008, the committee reserves the right to withdraw the prize and pay 50% of the net proceeds up to $20,000 to one winner.

5. Checks and or credit cards that are denied or returned for insufficient funds or other reasons will be subject to a $25 penalty and the ticket will be void until cash or a money order is received to cover said ticket.

6. The primary person named on the ticket will be awarded the prize and will be responsible for all taxes (Federal and State) incurred. No portion of the cost of the ticket is tax deductible. DSC is required by law to report winnings to the Internal Revenue Services and to withhold appropriate taxes on winnings. Winner must claim possession of the prize by November 17, 2008 or forfeit the prize.

7. Kemper CPA Group, LLP, an independent accounting firm, will conduct the official drawing of the winner on October 17, 2008 at Jupiters II, 6:00 pm. All board members, employees and volunteers of Developmental Services Center are eligible to win.

8. The winner, by entering, authorizes DSC to publish their name and likeness on the website and for it to be used in other promotional materials by DSC.

9. All sales of raffle tickets are final and there will be no refunds.

10. DSC does not rent or sell its donor or supporter list information to third parties.

11. Purchase of this ticket constitutes full agreement by the purchaser that he/she agrees with all terms and conditions set forth herein.

12. Void where prohibited by law.
