What is Case Management/Family Support Services?
DSC Case Management/Family Support Services provides to adults and children
with developmental disabilities a full range of services including advocacy,
referral, linkage, crisis intervention, and intermittent direct service, when
appropriate. Each consumer is assigned a case coordinator who monitors
treatment planning, implementation, and follow-through. As the single
point of contact for each consumer, the CM/FS staff are responsible for
coordinating the interplay of agency and community service providers.
Who may be served?
Champaign or Ford County residents with a developmental disability (mental
retardation, cerebral palsy, epilepsy, autism) who, due
to this disability, need services to enhance or maintain
his or her highest level of independence at home, work,
or in the community.
What services are available?
Case Management / Family Support Services include, but
are not limited to:
- Intake Screening
- A single point of contact
- Information and referral services
- Needs assessment
- Individualized habilitation plan development
- Coordination of services
- Treatment monitoring and follow-up
- Crisis intervention
- Service linkage
- Advocacy
- Sustaining support

Case Coordinators assist DSC consumers with service linkage such as Social Security, Public Aid, and other community agencies.

Respite
The Respite Program is designed to provide short-term care for individuals with developmental disabilities. Families are eligible for up to 180 hours of respite care per fiscal year (dependent upon availability of respite providers and funding). These hours can be used in times of crisis, planned vacations or other situations where short-term care is needed. Services are provided in the home of the individual or the respite provider's home.
Is there a fee?
There is no fee for DSC Case Management/Family Support Services. The
program is funded by the Illinois Department of Human Services.
