Meet Case Management!
DSC Case Management provides a full range of services to individuals with intellectual and/or developmental disabilities including advocacy, referral linkage, and crisis intervention. Each individual is assigned a Case Coordinator/QIDP (Qualified Intellectual Disability Professional) who facilitates planning, and implementation of an individual's desired outcomes. As the single-point-of-contact for each individual, the staff is responsible for coordinating the interplay of agency and community service providers. Another way we like to say this around DSC, is that Case Management is the “glue,” that holds the team together, serving as a liaison between an individual and/or family members, and all of the other services offered at DSC.
Case Coordinators/QIDPs (Qualified Intellectual Disability Professionals) are required by DHS (the Department of Human Services) to have at least one year of experience working with people with intellectual/developmental disabilities. They must also meet specific minimum federal educational requirements to be a QIDP.
